• Start gentle hydration
• Monitor I&O and watch for fluid overload
• EKG and cardiac monitoring
• CBC CMP UA, ABG?
• CT head without contrast 
• Echocardiogram 
• Carotid duplex 
• Check orthostatic vital 
• Discontinue offending agents/medication 
• PT OT

Treat underlying cause

• Carotid endarterectomy for severe carotid artery stenosis 
• Pacemaker for bradycardia 
• Aortic valve replacement for severe aortic stenosis 
• midodrine, salt and fluid intake in orthostatic hypotension 
• Reassurance for vasovagal syncope
