Duration depending on infectious syndrome and ANC recovery, ID consult, Gram-negative rods, Gram-positive cocci, anaerobics,
-cefepime 2 g IV Q 8 hour over 4 hour plus vancomycin per pharmacy dose if catheter or concern for pneumonia or SS TBI and plus metronidazole 500 mg IV Q 8 if mucositis or intra-abdominal, alternative aztreonam 2 g IV Q 8 hour plus vanco per pharmacy dosing plus metronidazole 500 mg IV Q 8
