  • Initial BP reduction goal:  In most clinical scenarios, the mean arterial pressure (MAP) should be reduced by ~10% within the first hour and a further 5%-15% over the next 23 hours.  Excessive abrupt decreases should be avoided because they can precipitate ischemia.  There are a few clinical scenarios (eg, intracerebral hemorrhage, aortic dissection) in which more aggressive lowering of blood pressure may be beneficial.
